Please provide the following Registration information, and be assured that your privacy is secure with Garden Harvest. We need these records as part of the volunteer waiver releasing Garden Harvest from any liability in case of injury during the time you volunteer.

Registration Donation	I will make my registration donation  at the Gate   My parents cannot afford a donation of even $1.00, nor do I have a sponsor, so I give Garden Harvest permission to use my name to obtain a sponsor who will donate $1.00 on my behalf.

WAIVER AGREEMENT

I wish to volunteer my services to alleviate hunger through Garden Harvest, Inc. Program. I understand that the nature of the volunteer activities which are typically performed by Garden Harvest volunteers, and which may be performed by one as a Garden Harvest volunteer, may involve strenuous physical activity, hot, cold, or otherwise inclement weather, working with unfamiliar persons, at times traveling to neighboring farms, possible exposure to ticks with Lymes Disease, poison ivy, poison oak, bees that sting, and other potential risks of injury associated with farm work and being around farm animals. The work varies according to the season.  It can consist of, is not limited to, planting, harvesting, watering, weeding, preparing fields by clearing brush & de-rocking, spreading mulch, fall cleanup, spring preparation. Knowing the risks inherent in farm activities, aforementioned & otherwise, I still wish to volunteer and hereby assume the risk, with respect to any liability of Garden Harvest for such risks of any accident or injury to my person or my property which I may sustain in connection with my participation as a Garden Harvest volunteer or in any Garden Harvest related activity. In addition, I hereby release and discharge Garden Harvest and any of the members, directors, officers, employees, partners, affiliates, agents, and successors from any and all liability or responsibility for any such accident or injury. Furthermore, I will not hold the person(s) who own and/or operate the farms(s) from which we glean, liable for accidents, injury, or death during the farming events. Furthermore, I do hereby grant and convey unto Garden Harvest all right, title, and interest in any and all photographic images and video or audio recordings made by Garden Harvest during my volunteer work.

When you hit the submit button, you will be presented with your confirmation Minor Volunteer Waiver Form, with the basic information on it.  Please have one of your parents sign and date this form and bring it with you the day you volunteer. Anyone under 18 years of age must have a signed Minor Volunteer Waiver Form before participating at Garden Harvest.   

If you absolutely cannot get this form to submit due to technical difficulties with this site, then please contact us to alert us of the problem, and then you can print out this printable volunteer waiver form, have your parents sign it, and bring it in.
